How do I take a company to small claims court after it has gone to a collection agency?

I have a gripe with T-Mobile over a bill. I have all documentation of the transactions and phone calls, but nothing was ever resolved and it went to collections. I've tried working with the agency to resolve, but they just want their money. I've gone through the Experian, Trans Union route, but that didn't resolve anything. I want this off my report and I need to know how to take it to small claims court. Thank
