How do I claim a settlement check from Mass lawsuit?

Wal-mart was sued for not giving breaks and cheating paychecks in Mass. A $40million dollar settlement was ordered for anyone who worked there between 1995 and the settlement date. So I can't find any way to actually get the check. Considering I have since married and moved have a dozen
